Please submit the narrative and financial reports, containing the information requested below, on the dates specified in your grant award agreement.  Reports should be submitted, preferably in electronic form, to your Hewlett contact.

The following questions are designed to assess the progress you have made toward our shared objectives as stated in your proposal.

Your report must not exceed five (5) pages in length. Do not bind or staple the report.

Please submit your report in English.

Please submit all accounting in U.S. Dollars (USD).
