Reporting Guidelines for Grants in the Performing Arts

In order to satisfy the Hewlett Foundation’s grant reporting requirements, please submit the narrative and financial reports by the dates specified in your grant award letter.

Once your interim reports are received, they must be approved by program and finance staff before the Foundation can release your grant payment installments. Please be aware that annual interim reports are required even if you received an advance payment due to extenuating circumstances. Final reports must be submitted at the completion of the grant period, as specified in your grant award letter.

If you are applying for a renewal grant, you must submit the final report along with your renewal application. We understand that the report will not cover the entire final year of the grant period.

Please include the grant number in each report and submit all reports by hard copy to Grants Administration, The William and Flora Hewlett Foundation, 2121 Sand Hill Road, Menlo Park, CA 94025.

If you received your grant in or after July 2003

The following questions are designed to assess the progress you have made toward our shared objectives as stated in your proposal.

A. Narrative Report

  1. Chart: In your proposal, you completed a chart in which you identified ultimate and intermediate outcomes, indicators, activities, and targets. Please add a column entitled “Progress” to that chart and fill in the new cells with measures of your progress toward your previously stated targets. Please attach your revised chart.
  2. Narrative: Please respond to the following:
    1. Describe the activities you conducted during the past year, and compare these activities to the expected activities you set forth in your grant proposal.
    2. Describe the progress you have made toward achieving the intermediate and ultimate outcomes you set forth in your proposal. Has your work resulted in any additional outcomes?
    3. Describe any major changes that have occurred during the past year within your organization or outside of your organization (e.g., in the community, the political landscape, etc.) that have had or will have an impact on the work supported by the grant. How have you responded to these changes, or how do you plan to respond to these changes?
    4. What lessons have you learned during the past year that will help you in your continued efforts to achieve your intended outcomes? Describe any alterations you have made or plan to make in light of these lessons.
    5. In your proposal, if you responded to the question of whether the organization plans to disseminate what it has learned from the work supported by the grant and from the evaluations undertaken in relation to this work, please provide an update with regard to the status of any such plans.

B. Financial Report

  1. Financial Statements: Please include a report that summarizes revenues and expenditures, and accounts specifically for the use of Foundation funds. Please also attach a statement of financial position (i.e., balance sheet) and a statement of activities (i.e., income statement) for the most recently completed fiscal year. If possible, these financial statements should be audited. If an audit is in progress, please attach unaudited versions of these statements and then forward an audited set once completed. If your most recently completed fiscal year audit is over four months old, please include the most recent month-end statements of financial position and activities.
  2. Additional Requirements for Project Support Grants: If your grant is for support of a particular project (i.e., if your grant award letter does not specify that the grant is for general operating support), please attach a schedule showing (a) actual expenditures to date funded by the grant, broken down by natural account (e.g., salary, benefits, travel, etc.); and (b) net grant funds not yet expended.
